Description of Position:

This position is responsible for providing direct evaluation, counseling and education services to individuals in treatment.
The work involves facilitating overall case planning for clients and their families and maintaining all necessary documentation of a client’s involvement in treatment. In addition this position also serves as a team leader for members of the substance abuse counselors, provides assistance to the Manager, and participates in the multidisciplinary team planning. Supervision is provided by the Manager of CADS Residential and Outpatient Services or Manager Riverside Residential and Outpatient Services.

To oversee all daily admissions and bed management within UnityPoint Health – St. Luke’s. The aim is to balance the access demands of the Emergency department and those of urgent/non-urgent elective work with available beds. The Patient Flow Coordinator will provide the clinical lead for a team that will include schedulers, validation and administrative personnel. The aim of this team is to optimize the continuum of care by managing the patient effectively throughout the whole of their care episode, from admission into the hospital and back. The process of enhancing patient management is accomplished by and safe admission/placement of patients entering the hospital and those who are transferring from the Critical Care areas, or within units of the hospital. Placing patients in the right bed efficiently as possible.

The CSU Crisis Clinician (LMHP) is supervised by the Manager, Outpatient Nursing Services Psychiatry and CSU and provides care to patients in crisis utilizing the Robert Young Center principles of Trauma Informed Care. The LMHP provides acute care to individuals who are experiencing a psychiatric crisis, and uses expertise to serve patients with complex behavioral health presentations that cannot be served in a behavioral health outpatient care setting. They perform crisis assessment, formulate a diagnosis, and implement acute care counseling interventions with patients. They often triage and prioritize care for multiple patients being referred for evaluation. They act as a patient advocate to coordinate required services or to resolve emergency problems in crisis situations. The LMHP assesses patients for risk of suicide attempts, plans treatment modalities, and coordinates treatment plans. In the crisis role, the LMHP consults with a medical provider to recommend the least restrictive plan of care. They also gather information about community mental health needs or resources and make referrals that could be used in conjunction with outpatient therapy. The LMHP often interacts with child protective services and works closely with county social workers and parents, collaborating on the plan of care. Patients are seen by the LMHP in person, via telehealth within multiple local Emergency Departments (ED), and may be assessed on inpatient units.
The LMHP is recognized as a Licensed Practitioner of the Healing Arts under Medicaid Community Mental Health Services Rule 132 and may provide clinical supervision to Mental Health Professionals (MHP) and may train students assigned in the department. The LMHP delivers customer service and works cooperatively with other departments, maintains accurate clinical documentation, and demonstrates Trinity’s principles and values when interacting with clients, families, visitors, physicians and coworkers.
