The Hospital Liaison has the opportunity to assist individuals who are engaged in hospital based services with appropriate outpatient care.  The liaison assesses patients while they are engaged in hospital-based treatment in order to connect them with ongoing treatment in the community.  The liaison also provides supportive services to assist the individual in accessing the necessary resources.

Responsibilities

  • Work within a comprehensive treatment team-inclusive of behavioral health professionals, psychiatric providers, medical staff
  • Coordinate with hospital-based mental health programs to assess patients
  • Utilize patient interview and hospital records to identify further treatment needs
  • Completes assessment and treatment planning to facilitate outpatient referrals
  • Link patients with mental health and/or substance use treatment following hospital discharge
  • Provides support and advocacy for patients when needed
  • Receive supervision hours for clinical licensure without cost, during you work
  • Position is based at Methodist hospital, services may be provided at other locations

Qualifications

Bachelor's degree in counseling or related field, or bachelor's degree in other field with 2 years documented clinical experience. Preferred master's degree in counseling or related field.
