Assessor and Liaison will provide engagement, and assessment activities on-site at area nursing facilities to support comprehensive William’s transition services. Develop and maintain relationships with area facilities and provide intensive follow-up. Complete intensive, comprehensive assessments through interviews with the individual, collateral information from the nursing facility, reviewing facility records, requesting documentation from previous providers, and speaking with family members, as appropriate. Responsible for coordinating the patient care and integrating both medical and psychiatric needs of the patient. Function as an effective team member and represent the agency in a professional manner.

Responsibilities

Completes assessments for transtion and co-ordinating care for medical and psychiatric challenges ,faciliate smooth transtion of resident from the Nursing to the community -80% and follows up with all UPH policies and team assigned meetings and tasks -20%

Qualifications

Masters degree in Social Work, Counseling, Marriage and Family therapy with Licensure and at least 2 years of Mental Health experience.
