Peer Support Specialist - ACT

FTE: 1.0 (Full-time, 40 hours per week)
Shift: various hours including days, nights, weekends

• Provides direct care to patients with a combination of mental health and basic living support to prevent the need for more intrusive level of care. Patient care will be provided at home or office, in community to monitor, supervisor or support activities of daily living.
• Functions as a practicing clinician on the team and provides service coordination (case management) for an assigned group of patients.
  • Coordinates and monitors activities of daily living.
  • Assumes responsibility for developing, writing, implementing, evaluation and revising treatment goals and plans in conjunction with client.
  • Provides transportation when needed and purposeful.
  • Supports patients with behavioral health symptom management.
  • Advocates for patients’ rights and preferences. Work cooperatively with referral agencies.
  • Help patient with paperwork and entitlement applications.
  • Document on contact with patients in EPIC.
• Participates in patient-centered comprehensive assessment within 30 days of new admission and once yearly on patients as assigned.
• Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
• Demonstrates ability to meet business needs of department with regular, reliable attendance.
• Employee maintains current licenses and/or certifications required for the position.
• Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
• Completes all annual education and competency requirements within the calendar year.
• Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.  Takes appropriate action on concerns reported by department staff related to compliance.
