Substance Use Disorder Counselor

Hamilton Outpatient - Human Service Center

1st Shift

 

Facilitates individual, couple, group, and family therapy to children, adolescents, and adults. Administers standardized assessment tools. Conducts comprehensive mental health assessments and demonstrate diagnostic ability of all Axis'. Facilitates consumer driven individualized treatment planning skills.

Responsibilities

  • Provides ongoing assessment of patients and providing counseling/mental health intervention according to person-centered treatment plan - 60%
  • Completes documentation within 24 hours - 20%
  • Provides collaboration and other interventions to provide effective care - 20%

Qualifications

Education:

  • Master's Degree in Counseling, Psychology, or Social work. 

License/Certification:

  • LCPC, LPC, LCSW, LSW preferred
  • Illinois Driver's License Required
  • Ability to pass Illinois Healthcare Workers Background Check
  • Must be able to work individually or in a team facility/hospital.

Must have control over personal use of all mood-altering chemicals and behaviors.

If recovering from an addiction, must currently have a minimum of 1 years of uninterrupted recovery and must actively treat this addiction by appropriate recovery practices.
