• Participate in treatment planning and assessments for residential consumer and assist consumers to work on their individual treatment plan goals.
• Provide medication observation, skills training, and maintain security of medication. Complete vitals, treatment and other procedures as ordered by physician under supervision of LPN/RN.
• Assess consumers and request supervisory, nursing, or emergency assistance.
• Assist with recovery based rehabilitation, behavior management, normalization and age appropriate treatment. Assist consumers to integrate into community by providing skill teaching support and monitoring.
• Meet productivity standards set for your department, and work shift as scheduled perform documentation requirements set by funding bodies.

Education:

• High School Diploma preferred

License/Certification:

• Direct Support Professional (DSP) Certification or Certified Nursing Assistant (CNA) preferred.
• Driver's license with an insurable driving record and vehicle are required.
