Community Support Specialist

Tazwood Center for Wellness

2nd Shift, Full-Time, Benefitted

Position purpose is to facilitate the development and enhancement of consumer strengths.


To assist consumer to meet their clinical and individual needs as they are identified.  To assist consumer in accessing and utilizing community and treatment services to enhance the consumer quality of life and independence.

Complete all clinical documentation and other paperwork/reports in a timely manner in compliance with governing body standards.


Implement crisis assessment and counseling as needed.  Participate in the assessment process as needed and required.


Work cooperatively with agency psychiatrist and treatment team regarding consumer treatment needs.  To advocate for consumers and refer to appropriate services based on consumer strengths, needs, and clinical status.


Provide ethical and recognized treatment modalities for consumer. Provide consultation to family and significant others involved with consumer as permitted under the Confidentiality Act, HIPAA, and all other regulatory agencies.






Requires basic computer skills and familiarity with a Windows environment.

Requires passing the IL Healthcare Workers Background Check.

Requires valid IL drivers licanse and insurable driving record.


Education /  Work Experience 

Prefer Bachelor’s degree in Behavior Health Field, Social Work, Psychology, Sociology, Human Service, or Criminal Justice.or related fields of study 

Or Bachelor's Degree in unrealated field with Required two years of work experience in Behavioral Health

Or Will accept Requried 5 years of work experience in Behavioral Health
