    UnityPoint at Home – Fort Dodge, Iowa
    Hospice Social Worker - LISW, MSW or BSW
    FTE: 0.4 (Part-time, 16 hours per week)
    Shift: Monday - Friday, 8am - 4:30pm

    Responsibilities

    • Maintains the standards of social work excellence in in accordance with the scope and practice and standards of care set forth by the proper state chapter, NASW Standards for Social Work Practice in a Health Care Setting, and the Social Work Code of Ethics.
    • Ensures quality and safe delivery of services to patients according to care plan.  Uses equipment and supplies safely, effectively, and efficiently.
    • Performs comprehensive psychosocial and behavioral  assessments, treatment plans and appropriate interventions and therapy for behavioral health illnesses and concerns.
    • Demonstrates ability to diagnosis behavioral health illnesses with use of DSM 5 and appropriate billing codes.
    • Conducts assessment, diagnosis and treatment of the social and emotional factors related to the patients/clients illness, need for care, response and adjustment to illness and care and completes appropriate patient-centered plan of care and interventions according to scope of practice.
    • Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.
    • Participates in the development of the plan of care and attends interdisciplinary team meetings.
    • Provides information and referral services for organization patients/families/caregivers regarding practical and environmental needs.
    • Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying.
    • Identifies and refers patient/family/caregivers to appropriate individuals or agencies as necessary.
    • Serves as liaison between patient/clients or family/caregivers and community agencies.
    • Participates in discharge planning when needed.  Assists patient/family/caregiver with securing durable power of attorney and with funeral arrangements as needed.
    • Maintains accurate and timely documentation of clinical records.
    • Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines.
    • Maintain compliance with Personnel policies and procedures.
    • Maintain regular and consistent attendance at work.
    • Behave in a manner consistent with all Corporate Compliance policies and procedures.
    • Perform other duties as requested by Clinical Manager to facilitate the smooth and effective operations of the organization.
    • Meets measures as required or determined by job productivity and performance improvement.
    • Participates in orientation and training of new employees or social work intern.
    • Achieve certification in Integrated Care Management within 1 year of hire. Certification includes completing the Integrated Care Management formal training session, successfully completing assigned computer based-learning modules within 2 weeks of training, and a competency evaluation. Use the Integrated Care Management philosophy, skills, tools and documentation in clinical practice. Demonstrate competency in selected ICM skills annually.

    Employee is responsible for completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UPAH programs and initiatives.

    Qualifications

    LISW QUALIFICATIONS REQUIRED:

    • Licensed Independent Social Worker/Licensed Clinical Social Worker – licensure in good standing as required by applicable law.
    • Master’s degree in social work with accreditation by the council on Social Work Education and current appropriate State License.
    • Valid Mandatory Reporter course completion by state(s) requirement
    • CPR Certification.
    • One year clinical experience in health care.
    • Strong verbal and written communication skills.
    • Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
    • Ability to work as a collaborative team member.
    • Ability to understand and apply guidelines, policies and procedures.
    • Valid licensed driver with automobile insurance in accordance with state and/or organizational requirements. 

    MSW QUALIFICATIONS REQUIRED:

    • Master’s degree in social work from a University/College accredited by the council on Social Work Education and State Licensure per State Code.
    • Valid Mandatory Reporter course completion by state(s) requirement
    • CPR Certification.
    • One year clinical experience in health care.
    • Strong verbal and written communication skills.
    • Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
    • Ability to work as a collaborative team member.
    • Ability to understand and apply guidelines, policies and procedures.
    • Valid licensed driver with automobile insurance in accordance with state and/or organizational requirements.

    BSW QUALIFICATIONS REQUIRED:

    • Bachelors’ degree in social work.
    • One year clinical experience in health care.
    • Strong verbal and written communication skills.
    • Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
    • Ability to work as a collaborative team member.
    • Ability to understand and apply guidelines, policies and procedures.
    • Valid licensed driver with automobile insurance in accordance with state and/or organizational requirements.
    • Valid Mandatory Reporter course completion by state(s) requirement
    • CPR Certification

    LISW/MSW/BSW CONTINUING EDUCATION REQUIREMENTS:

    All staff are expected to participate in appropriate continuing education. You are required to complete:

    • Mandatory Reporter: Child & Dependent Adult Abuse
    • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

     

    In addition, staff are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities.  All staff must attend mandatory educational programs.
