    Home Care Aide

    FTE: 1.0 (Full-time, 40 hours per week)
    Shift: Monday - Friday 8am-4:30pm with every 8th weekend


    • Ensures quality and safe delivery of services to patients according to care plan under the supervision of appropriate licensed personnel.
    • Uses equipment and supplies safely, effectively, and efficiently.
    • Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.
    • Assists with personal care and activities of daily living for patients (i.e. bathing, dressing, oral hygiene, skin care, etc.) and provides routine functions as assigned on care plan. Administers medication reminders as directed by Registered Nurse.
    • Available to work a flexible schedule and/or as needed per patient requirements.
    • Documents observations and care given each visit as designated by Registered Nurse.
    • Fulfills documentation requirements and uses appropriate tools effectively for patient care.
    • Communicates with appropriate licensed personnel (i.e., Registered Nurse I and II, Physical Therapist, Occupational Therapist, etc.) participating in care coordination to achieve high quality care.
    • Notifies Registered Nurse of any changes in patient’s behavior, condition or home environment.
    • Meets measures as determined and required for job productivity and performance improvement.
    • Maintain regular and consistent attendance at work.
    • Behave in a manner consistent with Mission, Vision, Values and Expectations for Excellence.
    • Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines.
    • Maintain compliance with Personnel policies and procedures.
    • Perform other duties as requested by the Manager to facilitate smooth and effective operations of the organization.
    • Assist with orientation of new staff as collaborative team member.
    • Employee is responsible for completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UnityPoint at Home programs and initiatives.



    • High School diploma or GED
    • CNA or graduate of 75 hour healthcare tech course.
    • Successfully completes organization competency evaluation program.
    • Strong interpersonal skills and ability to work as a collaborative team member.
    • Communicates effectively to ensure safety on the job and safe patient cares.
    • Sufficiently literate to communicate with patients, staff and others as job requires.
    • Ability to complete simple mathematical calculations for required clinical paperwork.
    • Licensed driver in the State of Iowa or Illinois with automobile insurance in accordance with state and/or organizational requirements.
    • Age specific care and reporting of Dependent Adult and Child Abuse training within one month of employment.
    • CPR Certification.
    • Valid Mandatory Reporter course completion by state(s) requirement
    • Valid licensed driver with automobile insurance in accordance with state and/or organizational requirements.
    • Maintains appropriate reciprocity for all states in which services are rendered



    • 2-5 years CNA experience and continuing education in the field.
    • Associates Degree and continued college education.



    All staff are expected to participate in appropriate continuing education. You are required to complete:

    • 12 hours of Aide Specific Training per year (3 per quarter)
    • Mandatory Report: Child & Dependent Adult Abuse
    • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
    • Accept personal responsibility for other educational activities to enhance job-related skills and abilities.
    • All staff must attend aide staff meetings as well as all mandatory educational programs.
    • Back Training Safety.
    • Other training and communication sessions as scheduled.
