  • Overview

    The Qualified Mental Health Professional (QMHP) Team Lead is supervised by the Clinical Manager, Community Support Program and provides care to clients utilizing the Robert Young Center (RYC) principles of Trauma Informed Care.  The QMHP Team Lead provides direction for a case management team, delivers comprehensive community based mental health services, and maintains accurate clinical documentation.  The QMHP oversees the clinical work of Mental Health Professionals (MHP) as required under Title 59: Mental Health Chapter IV: Department Of Human Services; Part 132 Medicaid Community Mental Health Services Program.  The QMHP may train students assigned to the department. The QMHP Team Leader demonstrates Trinity’s principles and values when interacting with clients, families, visitors, physicians and coworkers.

     

    UnityPoint Health Trinity, the State of Illinois, the City of Rock Island and the Development Association of Rock Island formed a unique partnership with the Robert Young Center to create one of the more innovative community support programs in the nation.

    The cornerstone of the Community Support Program in downtown Rock Island, Illinois, is District 22, a fresh perspective to the clubhouse model. The people being served in our Community Support Program are citizens who have diagnoses, such as schizophrenia and manic-depressive disorders. Currently, there are more than 400 people in the program ranging in age from 16 - 80.

    Responsibilities

    Clinical Leadership

    • Coordinates the multi-disciplinary process on behalf of the individual consumer for an assigned group of Mental Health Professionals (MHP) and/or Psychosocial Rehabilitation Program Coordinator in accordance with Rule 132.
      • Assigns referrals, reviews clinical activities, and may certify the diagnosis and recommended treatment plan.
      • Assures the timely and accurate completion of treatment plans, annual re-assessments, and daily progress notes.
    • Leads the development of the Individual Treatment Plan (ITP), reviews and signs the ITP, and completes within the established time frames and defined standards.
    • Ensures that all team members meet / exceed established productivity standards.
    • Provides clinical supervision
    • Provides quantitative information about each MHP’s performance to the CSP Clinical Manager for annual performance reviews.
    • Completes mental health intakes for CSP referrals.

    Care Coordination

    • Assures that all clients receive clinically sound triage/referral and ongoing services for behavioral health needs.
    • Performs diagnostic assessments and collects diagnostic information to determine an individualized treatment plan for the client.
    • Plans treatment modalities, coordinates treatment plans and follows clients through the treatment process

    Requests consultation for clients that includes but is not limited to providers, social workers, and nursing staff when necessary.

    • Evaluate the client’s treatment plan through follow-up assessment and makes adjustments based on clinical findings.
    • Identifies available support systems to meet the needs of each individual client.
    • Delivers high quality care appropriate to the age and educational level of the client.
    • Responds to crises situations for chronically mentally ill clients in community locations and the emergency department and provides emergency assessment, recommendations, and crises intervention.
    • Provides individual, family and group therapy
    • Participates in the RYC on-call schedule for emergency response after hours, weekends, and holidays.  

    Clinical Excellence

    • Maintains accurate, concise, current clinical documentation. This includes but is not limited to all phone conversations, requests from the consumer, changes in consumer condition, and all consultations with members of the health care team.
    • Integrates ethical principles, evidence based practice, and research findings into all areas of practice.
    • Establishes and maintains good working relationships with the medical, educational, and the social services community.

    Client Education

    • Utilizes a holistic approach to determine teaching methods appropriate to the individual client.
    • Provides education to consumers regarding their illness and how it relates to employment, and supplements information provided by health care professionals. 
    • Evaluate the client’s understanding of information provided and ability to follow through.

    Qualifications

     

    Minimum Requirements

    Preferred or Specialized

    Education:

    ·  An individual possessing a master's or doctoral degree in counseling and guidance, rehabilitation counseling, social work, psychology, pastoral counseling, family therapy, or a related field and has:

    Successfully completed 1,000 hours of practicum and/or internship under clinical and educational supervision; or

    One year of documented clinical experience under the supervision of a QMHP.

     

    Experience:

     

    Two years supervised experience in mental health or human services.

    License(s)/Certification(s):

    ·  Valid Illinois driver’s license.

    ·  Basic Life Support (BLS) certification.

    ·  Crisis Prevention Institute (CPI) training within 6 months of hire.

    ·  Obtain certification within 2 months of hire and re-certify annually according to the requirements set forth in the Illinois Department of Healthcare and Family Services related to the  Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS)

    Licensure in one or more of the occupations set forth by the Illinois Register Department of Healthcare and Family Services 89 Ill. Adm. Code 140:

    ·  A licensed social worker (LSW) possessing at least a master's degree in social work and licensed under the Clinical Social Work and Social Work Practice Act [225 ILCS 20] with specialized training in mental health services or with at least 2 years’ experience in mental health services; or

    ·  A licensed professional counselor possessing at least a master's degree and licensed under the Professional Counselor and Clinical Professional Counselor Licensing Act [225 ILCS 107] with specialized training in mental health services or with at least two years’ experience in mental health services; or

    ·  A registered nurse who holds a valid license in the state of practice, is legally authorized under state law or rule to practice as a registered nurse, so long as that practice is not in conflict with the

    Illinois Nurse Practice Act, and has training in mental health services or one year of clinical experience, under supervision, in treating problems related to mental illness, or specialized training

    in the treatment of children and adolescents; or

    ·  An occupational therapist who holds a valid license in the state of practice and is legally authorized under state law or rule to practice as an occupational therapist, so long as that practice is not in conflict with the Illinois Occupational Therapy Practice Act [225ILCS 75] with at least one year of clinical experience in a mental health setting. In the event the state of practice does not provide a legal authority for licensure, the individual must meet the requirements of 42 CFR 484.4 for an occupational therapist.

    ·  Demonstrated ability to apply critical thinking and problem-solving abilities to a wide range of client situations.

    ·  Able to accurately collect and assess information, understand ethical standards of behavioral healthcare, and utilize teaching/learning theories. 

    ·  Demonstrated excellent communication skills (orally and in writing) and presents self in a professional manner.

    ·  Able to establish effective consultative relationship with health care providers, outside agencies, co-workers, and the community. 

    ·  Able to work sensitively with clients. 

    ·  Able to handle multiple priorities, maintain composure under stress, and adapt to a fast paced changing environment.

    ·  Proficiency in the use of computer programs including Microsoft Office and Electronic Medical Record (EMR) applications
