Under the general supervision of the Manager of Rehabilitation Services, functions as a member of the Physical Therapy team; evaluates and establishes treatment programs for variety of patient diagnosis; treats patients in a variety of settings; provides patient/family education; performs necessary documentation; and supervises supportive personnel.

TECHNICAL/ADMINISTRATIVE/CLINICAL: (Total Weight of 100) Demonstrates competence in the skills, (processes, procedures, and equipment) necessary to carry out assigned duties as identified below. Demonstrated competence in recognizing and responding to patients physical, mental, emotional and developmental needs should be evaluated in relation to the patient populations served as appropriate for each responsibility.
A. Evaluates and plans treatment programs demonstrating competence in the administration of age appropriate care, for a variety of patient diagnoses and variety of settings, which may include but is not limited to the following:
B. Provides age appropriate care in accordance with established standards of care, policies, procedures, and state and federal law and summarizes treatment plan.
EDUCATION, TRAINING & EXPERIENCE: Bachelor's degree, Certificate or entry level Master's degree or Doctorate from an accredited Physical Therapy Program.
KNOWLEDGE, ABILITIES & SKILLS: Must be able to safely lift patients and operate various physical therapy equipment. Must be able to evaluate, plan treatment programs, and treat patients effectively. Must possess interpersonal skills necessary to deal effectively with patients, physicians, or other Trinity employees, outside agencies, and community.
LICENSES, REGISTRATIONS AND CERTIFICATIONS: Current IL and/or IA state license.
