ParticipatesinCenter's operations demonstrating professional behaviors to include supportinggoals of the Center and maintain awareness of resource utilization.
Provides clinical support to physician/ARNP/ PA for complex cases requiring case management, for example,contactingdoctors and other health care providersto coordinate services.
Works cooperatively with referral agencies in coordinating services to ensure optimal consistency of serviceprovisions.
Maintains and develops professional relationships with all outside agencies and external customers bycommunicating in an efficient, professional and courteous.
Demonstrates positive customer.
Maintainscompliance with Human Resources policiesand procedures including the dress code.
Preferred Associates or Bachelors degree in Social Work, Human Services, or another Human Service related field.
One (1) to two (2) years preferred experience within a behavioral health setting.
Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively.
Must demonstrate knowledge and skills required for specialized area of assignment.
Must demonstrate knowledge and skills necessary to provide care appropriate to adult and geriatric patients.
Must demonstrate effective communicationskills, i.e., demonstrates effective written and oral communication skills for use with physicians, co-workers, other clinic/hospital personnel, as well as the public.
Requires good organizational skills.
Must be able to objectively perform initial assessment of patient’s condition along with needs related to adult and geriatric behavioral health patient.
Must possess the ability to effect planned change in apositive manner.
Ability to work effectively with others, think creatively and communicate effectively,
Requires working knowledge of community agencies. Utilizes computer documentation effectively.
Must have a valid driver’s license, auto insurance and reliable transportation.
