  • Overview

    • To provide expertise and clinical oversight in the Home Care referral management process.

    Responsibilities

    Assembles all information regarding potential clients referrals including client's medical and financial data to ensure client admission to service.

    • Documents accurate and appropriate information in a legible manner for use in the admission process to home care.
    • Ensures client needs appear to meet the acceptance policies for a Medicare-certified home health agency.
    • Enters admission data into the Home Health automation system.
    • Schedules admission visit per agency policy or physician's order.
    • Collects as much information as possible from the referral source including, via, interview, and inquires into the hospital's automation system.
    • Obtains financial approval for all insurance and IDPA clients at the time of referral per payer agreements.
    • Communicates self pay portions to clinical staff in writing
    • Provides necessary documentation and communication to the Patient Account Specialist to ensure accurate and timely payment of services provided.
    • Educates clinical staff regarding necessary data collection and documentation needed to ensure payment.
    • Serves as an advocate for the client in obtaining the most appropriate home care services available to them.
    • Communicates to the client/caregiver, as appropriate, any and all information regarding the referral to Home Health.
    • Assists clinician in obtaining additional requested information to improve client care delivery.
    1. Serves as a Home Health liaison and marketer for various health care customers and clients.
    • Coordinates communication between disciplines and field staff when the primary RN is unavailable.
    • Documents communications for inclusion in the medical record.
    • Educates and markets the services that Methodist Home Health Services can provide to external customers and community services.
    • Actively participates in the care coordination of clients involved with other outpatient facilities including, but not limited to, Heartland Infusion Therapy Services (HITS), Wound Clinic, Professional Therapy Services (PTS) and inpatient facilities.
    • Participates in educational opportunities with new employees and ongoing educational offerings within the organization and community.
    • Communicates appropriate information to all participating care providers, as necessary.
    • Facilitates communication with inpatient case managers regarding current clients, as necessary.
    1. Coordinates the care management of insurance and IDPA clients for appropriate continuation of care per payer class arrangements.
    • Obtains financial approval for all insurance and IDPA clients throughout the certification periods as defined by the payer.
    • Provides ongoing reports and communications , written and verbal, regarding all clients as required by the payer source.
    • Keeps appropriate documentation on case discussions with payer sources.
    • Collaborates with clinical staff and other care providers, as needed, to ensure financial coverage of services provided to the client.
    • Communicates to the business manager any financial situations that may be compromising to the agency or client.
    1. Actively participates in the acquisition of physician orders and enters data for care delivery.
    • Accepts and documents physician phone-called orders per policy.
    • Communicates orders, in writing and/or verbally, to appropriate staff including Scheduling, nursing, PTS, other disciplines and/or next care provider.
    • Adjusts schedules, as needed, to accommodate client needs.
    • Verifies transcribed orders are accurate.
    • Communicates any changes made to printed schedules to appropriate parties.
    • Provides next-day schedules to clinical staff, per request, via telephone.
    • Provides comprehensive scheduling responsibilities in the absences of the Scheduling Clerk and/or on Saturdays

    Qualifications

    • Education
    1. Bachelor's Degree in the field(s) of Nursing or Health related field required preferred
    • Work Experience
      1. 3-5 years of experience in/with acute nursing background required.
      2. 3-5 years of experience in/with home health required.
    • License and Certification
      1. CPR
      2. Registered Nurse in the state of IL
      3. Valid Driver’s License in the state of IL
