SPECIALTY NURSE – HEART FAILURE COORDINATOR
Applicant will work closely within The Heart Failure Program providing advanced heart failure nursing care to program patients. This includes the following; 1) providing patient and family education in home health surveillance techniques and optimal medication delivery systems; 2) optimizing adherence to pharmacologic and nonpharmacologic heart failure therapies by identifying and overcoming adherence barriers; 3) identifying and cultivating patient's support systems; providing nurse initiated telephone follow-up and answering patient initiated phone contacts; and 4) adjusting standard heart failure medications using defined physician driven protocols. These activities will be done in the setting of routine clinic visits, unscheduled clinic visits, outpatient intravenous infusion visits and prior to acute hospital discharge (to optimize the transition of care from inpatient to outpatient domains).
