This position provides assistance to the management team and staff of the Case Management/Care Transitions Departments at all campuses for UnityPoint Health. In addition the incumbent functions as the liaison between the external customers and the department and provides office management duties including (but not limited to) inventory control, scheduling of meetings and supply ordering. This position also provides clerical and secretarial support in a timely and efficient manner. The incumbent provides automated computer support in the areas of word processing, graphic design, spreadsheets, and form design. The incumbent collects and analyzes data related to department processes in order to determine opportunities for improvements and or system changes.
With direction, assists the Case Management/Care Transitions team in the implementation of discharge plan to SNF/NH including documentation and logistical arrangements or equipment necessary to assist in facilitation of post-acute discharge plan for patients.
Assists with Data Management processes
Provides team member assistance to the department management team and team members in the areas of secretarial support.
