This position provides assistance to the management team and staff of the Case Management/Care Transitions Departments at all campuses for UnityPoint Health. In addition the incumbent functions as the liaison between the external customers and the department and provides office management duties including (but not limited to) inventory control, scheduling of meetings and supply ordering. This position also provides clerical and secretarial support in a timely and efficient manner. The incumbent provides automated computer support in the areas of word processing, graphic design, spreadsheets, and form design. The incumbent collects and analyzes data related to department processes in order to determine opportunities for improvements and or system changes.


With direction, assists the Case Management/Care Transitions team in the implementation of discharge plan to SNF/NH including documentation and logistical arrangements or equipment necessary to assist in facilitation of post-acute discharge plan for patients.

  • Assists all team members of the Case Management/Care Transitions team in discharge planning matters.
  • Facilitates authorizations for transition to the next level of care including but not limited to: acute/subacute and intermediate care facilities including transportation.
  • Works with financial counselors to assist patients with Medicaid, Medicare, and other financial resources when necessary.
  • Maintains professional documentation as defined by organizational policy and procedure.

Assists with Data Management processes

    • Assists with data entry
    • Assists with extracting data from database and compiling for review and presentation.
    • Assists with identifying trends through tracking and trending processes
    • Able to work with multiple departments within UnityPoint Health system to assist with NGACO data related to SNF
    • Collects and maintains databases related to the utilization review process, including RAC/denial management activity, in order to determine opportunities for improvements and/or system changes.
    • Maintains a log of Patient Assistance Fund expenditures, H.S. I correspondence and others as requested.

Provides team member assistance to the department management team and team members in the areas of secretarial support.

    • Types memorandums, policy and procedures, minutes, newsletters, manuals, forms development, and other correspondence.
    • Responsible for scheduling meetings, typing agendas, and sending out meeting notices for the Department as requested.
    • Maintains the inventory of all department supplies and equipment and is responsible for the ordering of the supplies.
    • Oversees the ordering of approved subscriptions, and other reference materials required for the Department.
    • Prioritizes and completes assignments independently and in a timely manner, able to meet urgent deadlines successfully.
    • Responsible for collecting and reporting payroll time records for Department.
    • Responsible for making travel arrangement for Department staff.
    • Assists in maintaining the Community Resources folder.



  • Associate's Degree Preferred.


  • 1-2 years experience with word processing, spreadsheets, graphic displays, phone reception and other clerical duties required. 1-3 years of experience in medical field, utilization management or quality improvement preferred.


  • Valid driver's license when driving any vehicle for work-related reasons. LPN or Medical Office Assistant preferred.
