Overview

This position serves as the care coordinator for assigned cancer patients.  this position will follow and collaborate with patients from the time of diagnosis through survivorship.

Responsibilities

Oversees a caseload of patients across the care continuum to include pre and post treatment management in collaboration with the care team 

  • Collaborates with care team and provider to assist in process of care adherence/quality assurance during the inpatient stay and through the continuum of care
  • Acts as a liaison between all support services
  • Enhances patient access and ease of referral and management 
  • Ensures that the patient is connected to prevention and treatment services
  • Ensures patient receives education and resource materials
  • Executes patient support and monitoring in post acute care environment
  • Collaborate with Bundled Payment project to assist in meeting project objectives
  • Provides specialized oncology experience acting as a resource and educator to patients, caregivers, and staff.
  • Assist with data entry; performance goal tracking
  • Assist with scheduling appointments for APN

Facilitates communication between the provider and the multidisciplinary team

  • Schedules patient for additional appointments/tests/exams as needed/required
  • Ensures pertinent results, plans of care are prepared and provided to the patient to  facilitate care coordination and communication
  • Collaborates with provider to ensure that information conveyed to patients is accurate and appropriate
  • Facilitates patients scheduling for multi-disciplinary treatment conference and ensures all related materials/test results are available
  • Collaborates with other members of the care team such as primary care and support services
  • Able to establish and maintain relationship networks with internal and external departments and services

Faciliates a quality treatment experience and outcome for the patient

  • Provides specialized oncology experience acting as a resource and educator to patients, caregivers, and staff
  • Serves as a patient advocate working closely with the provider, the care team and patient support systems to connect patients with needed services throughout the continuum of care
  • Assists with preparation, delivery, and education of survivorship care plan
  • Works to promote a positive work environment which emphasizes respect for patients, families, physicians, internal and external departments, and staff
  • Collects and documents required outcome metrics

Works collaboratively with oncology servcie staff to ensure ongoing program development and improvement with outcomes and satisfaction.

  • Continually networks with other community and professional programs to seek best practices
  • Prepares reports for monitoring program outcomes
  • Assists in meeting facilitation as assigned

Qualifications

Education

  • Bachelor's degree in the field of Nursing or the completion of such within 5 years is required

Work Experience

  • 3-5 years of experience in/with the care of Cancer Patients Required

License and Certification

  • Licensed as Registered Nurse in Illinois required

  • Intermediate computer knowledge: Ability to troubleshoot minor problems within a Windows OS, operates within a network environment, uses spreadsheet, database, word processing and internet applications proficiently. Learns new applications without difficulty and is able to aid others in immediate work area with computer questions
