UnityPoint Health- Trinity is accepting applications for the Care Coordinator for the Robert Young Center in Cedar and Muscatine County. No health care system is complete without a behavioral health division. The recent advances in behavioral health have given way to richer, deeper, treatment programs that put the patient at the center of each care plan. Since our founding, the Robert Young Center has championed mental health care advocacy, offering services under UnityPoint Health - Trinity


Under the direct supervision of the Director of Integration the care coordinator will provide comprehensive care coordination services for adults and children




Demonstrates competence in the skills, (processes, procedures, and equipment)  necessary to carry out assigned duties as identified below. Demonstrated competence in recognizing and responding to patients physical, mental, emotional and developmental needs should be evaluated in relation to the patient populations served as appropriate for each responsibility.


Principle Duties and Responsibilities of Care Coordinator

  1. Provides assertive community outreach and sustained efforts to engage individual clients.
  2. Collects information necessary to develop an accurate assessment of each client
  3. Provides emergency assessments of persons with mental illness in the community. Accurately assesses patients for suicidal gestures and ideation.
  4. Assists client to complete the first three steps of a Wellness Recovery Action Plan (WRAP), including understanding his/her illness and developing action plans for maintaining wellness, identifying early signs and triggers of relapse, and identifying signs of crisis.
  5. Assists in the timely development of the initial treatment (IT) plan and monitors the client’s progress toward achieving their patient-centered, patient-driven goals
  6. Assists clients in the development of crisis plans.
  7. Provides linkage to other mental health services and programs including psychiatric services, medication management, transportation assistance,
  8. Assists the client in securing basic necessities ( e.g. food, housing income, medical care)
  9. Provides counseling, problem-solving and personal support. Engages the client’s family and personal support systems.
  10. Teaches and assists with activities of daily living in home and in the community. Uses cognitive rehabilitation, in-vivo skills practice and role rehearsal (psychosocial rehabilitation).
  11. Completes all necessary documentation in a timely and accurate manner and in compliance with established standards of the State of Iowa and the Robert Young Center.
  12. Provides disease management and holistic patient care. 


Additional Principle Duties and Responsibilities of an Emergency Room Department Care Coordinator (Muscatine and Eastern Iowa)

  1. Assists with tele-psychiatry process in the emergency room setting, providing collaboration and patient support using LEAN concepts, standard work flow and huddle participation with Emergency Room Team.
  2. Work with ED staff on discharge planning for patients who access emergency psychiatric services.
  3. Completes all necessary documentation in a timely and accurate manner and in compliance with established standards of the State of Iowa and the Robert Young Center. 
  4. Identifies patient’s health care needs, connecting them to the appropriate services in the community.  
  5. Establishing wrap around services that promote stabilization. Connecting the patient to appropriate primary, behavioral and community resources that are patient driven.
  6. Ensures that each patient has a follow up appointment with a provider.
  7. Provides follow up phone call after an emergency room visit within 72 hours to ensure patient understands and has the means to follow discharge plan
  8. Completes Crisis Stabilization Plan for behavioral health patients 
  9. Communicates daily with the community care coordinators, performing a warm handoff and report to them so that they are able to implement and follow the patients discharge plan.  






EDUCATION, TRAINING & EXPERIENCE: Requires a BA, BS, BSW in behavioral healthcare services related field


KNOWLEDGE, ABILITIES & SKILLS: Demonstrates an ability to apply critical thinking and problem-solving abilities to a range of client situations; has the ability to accurately collect and assess information; can develop treatment plans and interventions with clients; possesses interpersonal skills, professional attitude and understands the ethical standards of behavioral healthcare. Is able to recognize signs of suicidal behavior and assess for lethality.
