This position will provide whole-person care coordination across medical, behavioral, and social service community systems.
The Care Coordinator’s primary responsibility is to respond to the needs of adults with a Serious Mental Illness (SMI) or Children with Serious Emotional Disturbance (SED) using a patient-centered approach which removes silos of care and supports an integrated system.
Bachelor Degree in Human Service Field
Valid driver's license
Write, read, comprehends, and speaks fluent english
One year experience working with adults and/or children with mental illness preferred
Used of usual and customary equipment used to preform essential functions of the position
