Overview

Responsible for the delivery of patient cares through the nursing process of assessment, diagnosing, planning, implementation, and evaluation.  Responsible for directing and coordinating all nursing care based on established clinical nursing practice standards.  Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.  Supports the organization vision and mission.  Utilizes the knowledge of patient’s age and cultural diversity into the provision of patient care.  Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care.   

Responsibilities

Delivers nursing care according to the standards of care devised within the American Nurses Association (ANA) Standards of Clinical Practice and the Emergency Nursing Association (ENA) Standards of Practice as appropriate. 

  • Collect patient health data as per Admission Assessment Policy 
  • Determine the priority of patient’s immediate conditions or needs 
  • Collect data utilizing the appropriate assessment techniques 
  • Involve the patient, significant others and healthcare provider’s in assessment when appropriate 
  • Ensure that all assessments are done in a systematic and on-going process 
  • Document relevant data in the medical records according to hospital standards.      

Systematically evaluate the quality and effectiveness of nursing practice 

  • Participate in Performance Improvement activities 
  • Assist with policies and procedures 
  • Analysis of performance improvement to identify opportunities for improvement and/or to initiate change      

Supports and documents the organization’s Infection Control Program 

  • Demonstrates adherence to infection control policies as evidenced by annual in-service records and employee practices 
  • Demonstrates knowledge of appropriate personal protective equipment and uses same in practice 
  • Notifies the Infection Control Practitioner of real or potential exposures 
  • Demonstrates knowledge of OSHA regulations 
  • Complies with OSHA regulation regarding Bloodborne Pathogens Exposure Control Plan 
  • Complies with OSHA regulation regarding TB Exposure Control plan 
  • Demonstrates knowledge of organizations’ Hazardous Waste Management Plan 
  • Discards waste in appropriate bags, receptacles, etc.    

Supports and promotes the organization’s Safety Program 

  • Demonstrates adherence to safety policies as evidenced by annual in-service attendance records and employee practices 
  • Maintains a clean and safe environment for patients and co-workers 
  • Follows all general safety rules  
  • Identifies the person designated as Safety Officer 
  • Notifies the Safety Officer or designee on-call of real or potential safety hazards 
  • Demonstrates an understanding of fire drills, disaster drills, internal/external evaluation plans and procedures as evidenced by in-service records and/or participating in drills 
  • Demonstrates knowledge of Environment of Care Committee activities 
  • Demonstrates knowledge of Disaster Sub-Committee activities      

Supports and promotes the organization’s Risk Management Program 

  • Demonstrates adherence to risk management policies as evidenced by in-service attendance records and employee practices 
  • Submits incident reports according to hospital policy 
  • Notifies the Risk Manager of incidents involving injury or having the potential for injury 
  • Maintains confidentiality regarding knowledge of adverse incidents      

Decisions and actions on behalf of patients are determined in ethical manner 

  • Maintain patient confidentiality 
  • Assumes the role of patient advocate 
  • Delivers care in a nonjudgmental and nondiscriminatory manner that is sensitive to diversity 
  • Delivers care in a manner that preserves/protects patient autonomy dignity and rights    

Contributes to the professional development of peers and other healthcare providers 

  • Shares knowledge and skills with others 
  • Delegation of duties to peers and associates      

Analyzes the assessment data in determining diagnosis/patient problems 

  • Derives diagnosis/patient problem from the assessment data 
  • Validates assessment data 
  • Documents diagnosis/patient problem in a manner that facilitates the determination of expected outcomes and plan of care      

Identify expected outcomes individualized to the patient.  Document outcomes as measurable goals 

  • Formulates outcomes mutually with the patient and healthcare providers when possible 
  • Ensures outcomes are realistic in relation to the patient’s present and potential capabilities 
  • Ensure outcomes provide direction for continuity of care and referral to social services as appropriate 
    • Implement the interventions identified in the plan of care 
    • Evaluate the patient’s progress toward attainment of outcomes 
    • Shift to shift review of narcotic sign-out shall be performed prior to off-going shift leaving 
    • Ensures fiscal responsibility when reviewing staffing guidelines for the on-coming shift.  Staffing guidelines are systematically reviewed with acuity of patients, staffing mix and matrix established by Administration.  Consults with Nurse Manager and Director of Nursing for unit needs 
  • Demonstrates ability to adapt to varied age-specific patient populations 
  • Supports the philosophy, objectives and goals of personal and professional growth 
  • Acquires and maintains current knowledge in nursing practice 
    • Participates in ongoing educational activities as appropriate 
    • Identification of aspects of patient cares at daily morning conferences 
    • Formulation of recommendations to improve nursing care at daily morning conferences 
    • Implementation of activities to enhance the quality of nursing practice  

 

Performs other related duties as assigned

Qualifications

Minimum Education/Certification/Registration

  • Associates Degree in Nursing
  • Registration and Current RN License in State of Iowa
  • CPR Certification
  • ACLS
  • Minimum Experience
  • Sufficient previous clinical experience in area of expertise
