Overview

Under the direct supervision of the Director of Integration the care coordinator will provide comprehensive care coordination services for adults and children

 

(3-4 days Muscatine, 1-2 Scott County)

 

Responsibilities

 

TECHNICAL/ADMINISTRATIVE/CLINICAL: (Total Weight of 100) Demonstrates competence in the skills, (processes, procedures, and equipment) necessary to carry out assigned duties as identified below. Demonstrated competence in recognizing and responding to patients physical, mental, emotional and developmental needs should be evaluated in relation to the patient populations served as appropriate for each responsibility.

 

Principle Duties and Responsibilities of Care Coordinator

  • Provides assertive community outreach and sustained efforts to engage individual clients.
  • Collects information necessary to develop an accurate assessment of each client
  • Provides emergency assessments of persons with mental illness in the community. Accurately assesses patients for suicidal gestures and ideation.
  • Assists client to complete the first three steps of a Wellness Recovery Action Plan (WRAP), including understanding his/her illness and developing action plans for maintaining wellness, identifying early signs and triggers of relapse, and identifying signs of crisis.
  • Assists in the timely development of the initial treatment (IT) plan and monitors the client’s progress toward achieving their patient-centered, patient-driven goals.
  • Assists clients in the development of crisis plans.
  • Provides linkage to other mental health services and programs including psychiatric services, medication management, transportation assistance 
  • Assists the client in securing basic necessities ( e.g. food, housing income, medical care) 
  • Provides counseling, problem-solving and personal support. Engages the client’s family and personal support systems. 
  • Teaches and assists with activities of daily living in home and in the community. Uses cognitive rehabilitation, in-vivo skills practice and role rehearsal (psychosocial rehabilitation). 
  • Completes all necessary documentation in a timely and accurate manner and in compliance with established standards of the State of Iowa and the Robert Young Center. 
  • Provides disease management and holistic patient care.

Duties of Care Coordinator for Eastern Iowa- See duties under 1. above.

  • CC receives referral from ED CC, CSU, IHH ED admission list etc.
  • CC makes phone contact within 72 hours (preference within 24 hours) after discharge from inpatient or ED.
  • CC explains Eastern Iowa Program and offers to schedule face to face appointment.
  • CC meets with client within 7 days of discharge and discusses recent ED/inpatient admission and assists with scheduling 7 day follow up with provider. CC completes Health Risk Assessment if appropriate.
  • CC schedules another follow up appointment to complete Crisis plan, assist with closing gaps in care identified in Health Risk Assessment.
  • Staff continues to provide face to face/phone contact with client for 30 days.

 

Qualifications

 

EDUCATION, TRAINING & EXPERIENCE: Requires a BA, BS, BSW in behavioral healthcare services related field, or a practical nurse (LPN) licensed under the Nursing and Advanced Practice Nursing Act (225 ILCS 65) with 2 years supervised experience in mental health or human services, or a certificate of psychiatric rehabilitation from a DHS approved program plus a HS diploma and 2 years experience in providing mental health or human services, a recovery support specialist certified and in good standing with the Illinois Alcohol and Other Drug Abuse Professional Certification Association, plus one year experience providing mental health services, or minimum of five (5) years supervised experience in mental health or human services and a HS diploma.

 

KNOWLEDGE, ABILITIES & SKILLS: Demonstrates an ability to apply critical thinking and problem-solving abilities to a range of client situations; has the ability to accurately collect and assess information; can develop treatment plans and interventions with clients; possesses interpersonal skills, professional attitude and understands the ethical standards of behavioral healthcare. Is able to recognize signs of suicidal behavior and assess for lethality.

 

LICENSES, REGISTRATIONS AND CERTIFICATIONS: Valid Driver's license.
