Join our Emergency Department team as an RN Care Coordinator! In this role, you’ll coordinate patient care across the acute care continuum by partnering with physicians, nursing, social services, and other care teams. You’ll support care planning, assess patient needs, monitor progress, and help ensure safe transitions of care through discharge planning and community referrals. You’ll also educate patients and families, address barriers to care, and serve as a key communicator to promote continuity, reduce readmissions, and improve patient outcomes.

• Assess patients to identify clinical, psychosocial, financial, and other barriers that may impact recovery and discharge.
• Develop and coordinate individualized plans of care based on patient needs.
• Collaborate with patients, families, physicians, nursing, social work, payers, and community agencies to ensure safe transitions of care.
• Coordinate post-acute services and resources, including home care, equipment, medications, therapies, and follow-up appointments.
• Monitor patient progress, length of stay, avoidable days, and potential barriers that could lead to delayed discharge or readmission.
• Educate patients and families on treatment plans, medications, self-management, discharge instructions, and available community resources.
• Maintain timely and accurate documentation while supporting regulatory requirements, quality initiatives, and continuity of care.

Education:

• Graduate of an accredited nursing program (ADN or nursing diploma) required.

• BSN preferred.

Experience:

• Three (3) years of clinical RN experience in an acute care or hospital setting required.

• Care coordination, case management, utilization management, or discharge planning experience preferred.

• Experience with the patient population served preferred.

License(s)/Certification(s):

• Valid RN license in the state(s) of practice required.

• BLS and ACLS preferred.

• Case Management or related specialty certification preferred.

• Required state-specific mandatory reporter training must be completed within 90 days of hire.
