As a Licensed Practical Nurse (LPN), you’ll be a valued member of the Acute Care team, working alongside Registered Nurses and other healthcare professionals to provide compassionate, evidence-based care to patients and their families. • Provide hands-on nursing care for patients with physical disabilities and chronic illnesses.
• Assess patient needs and contribute to individualized plans of care, education, and discharge planning.
• Educate patients and families and reinforce care and instructions provided by other team members.
• Provide direction and supervision to unlicensed assistive personnel while delegating appropriately within the LPN scope of practice.
• Collaborate with nurses, providers, and interdisciplinary team members to coordinate patient care and achieve overall goals.
• Communicate effectively with patients, families, and healthcare team members while maintaining accurate documentation.
• Implement the interdisciplinary plan of care to promote recovery, maintain function, and prevent complications or further loss of function.

Education: 

• Graduate of accredited LPN program required.  

Experience: 

• One (1) year of previous CNA/PCT experience preferred.  

License(s)/Certification(s): 

• Current licensure in good standing to practice as a Licensed Practical Nurse in primary state. Must apply within 90 days for licensure in secondary state. 

• Must possess and maintain current Basic Life Support (BLS) certification or obtain  

  certification within three (3) months from date of hire.
