The RN Hospitalist is a registered nurse who collaborates with hospitalists, unit staff, and the interdisciplinary team to coordinate and manage inpatient care from admission through discharge and handoff to outpatient providers. The role synthesizes assessment findings to support planning, implementation, and evaluation of care; promotes efficient use of resources; and ensures safe, timely transitions, including patient/family education, follow-up appointments, and linkage to post-acute services. The RN identifies patients with complex medical or social needs and helps develop strategies to reduce readmissions and improve outcomes. Success in this role requires strong clinical judgment, communication, and organization skills, a commitment to service/logical collaboration, and practice within the scope of licensure under nursing and hospitalist leadership.
