Overview

Join a mission-driven oncology team as an RN Navigator, guiding patients from diagnosis through survivorship as their primary point of contact. In this role, you’ll coordinate care across the continuum, identify and reduce barriers to timely treatment, and support patients and families through education and resource connection. Partnering with a multidisciplinary team, you’ll develop individualized care plans, monitor quality and guideline adherence, and help improve outcomes, experience, and efficient use of resources.

Responsibilities

  • Coordinate end-to-end cancer care navigation, ensuring timely access to consultations, diagnostics, treatment, and follow-up services across the continuum. 
  • Partner with multidisciplinary oncology teams to streamline care coordination and participate in tumor boards and case conferences. 
  • Serve as the primary patient and family liaison, providing education, advocacy, and support throughout diagnosis and treatment. 
  • Assess patient needs, address barriers to care, and connect individuals with social, financial, psychosocial, and community resources. 
  • Deliver personalized education on diagnoses and treatment options, ensuring patient understanding and shared decision-making. 
  • Facilitate seamless communication among physicians, departments, and external partners to optimize care delivery and continuity. 
  • Develop and maintain patient tracking systems, ensuring accurate documentation, data integrity, and timely reporting. 
  • Monitor quality indicators and participate in continuous improvement initiatives to enhance clinical outcomes and patient experience. 
  • Collaborate on development and implementation of evidence-based oncology programs, protocols, and patient education materials. 
  • Ensure compliance with regulatory standards while promoting coordinated, patient-centered, and equitable access to cancer care. 

Qualifications

Education: 

  • Graduate of an accredited nursing program; Associate Degree in Nursing required. 
  • Bachelor of science in nursing preferred.  

Experience: 

  • Three (3) years of registered nursing experience is required 
  • Two (2) years of oncology nursing experience (medical, surgical, or general) preferred.  

License(s)/Certification(s): 

  • Valid RN license in the state(s) of practice, including eligibility under a NLC multistate license required 
  • Dual licensed positions must obtain and submit proof of dual licensure within 90 days of hire/transfer; proof of application is acceptable during this period.  
  • Must possess and maintain current Basic Life Support (BLS) through the American Heart Association. 
  • Certification in Oncology or Navigation that meets accreditation requirements required within two (2) years of hire: 
  • Advanced Oncology Certified Clinical Nurse Specialist - Oncology Nursing Certification Corporation (ONCC) 
  • American Cancer Society Leadership in Oncology Navigation- American Cancer Society Oncology (ACSO) 
  • IL & IA Child Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire. 
  • IL & IA Adult Dependent Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire.
