PACE (Program of All-Inclusive Care for the Elderly)

Monday-Friday 8:00AM-4:30PM

 

Under the supervision of the PACE Transportation/Facility Manager, is responsible for providing transportation services utilizing agency vehicle for PACE participants according to assigned routes/appointment schedules.  Provide supportive services to ensure the safety of our frail elderly participants.  Provides supply/equipment delivery services as needed.

Responsibilities

  • Provide safe, reliable transportation for PACE participants to appointments and program activities while delivering exceptional customer service.  
  • Assist participants with mobility needs, including wheelchair transport, ambulation, transfers, and home access.  
  • Monitor participant well-being, communicate changes in health or home conditions, respond appropriately to emergencies, and help create a welcoming, supportive environment.  
  • Follow transportation schedules, safety procedures, and all traffic laws while working independently and collaboratively with the care team. 
  • Maintain vehicles in a safe, clean, and ready-to-operate condition through daily inspections, fueling, and routine safety checks.  
  • Monitor vehicle performance, report maintenance needs, and support reliable transportation services across the PACE program.  
  • Participate in an after-hours on-call rotation as needed. 

Qualifications

Education: 

  • High School graduate/GED required, or equivalent work experience. 

Experience: 

  • One (1) year of experience working with the frail or elderly population preferred.  
  • At least six (6) months experience transporting individuals with special mobility needs strongly preferred. 

License(s)/Certification(s): 

  • Must possess and maintain current Basic Life Support (BCLS) within 30 days of hire.  
  • Valid driver’s license (Chauffer’s Endorsement for IA license if determined necessary by Market), and good driving record required 
  • IA Child Abuse Mandatory Reporter training in state(s) where providing care required within 60 days of hire. 
  • IL Child Abuse Mandatory Reporter training in state(s) where providing care required within 60 days of hire.  
  • IA Dependent Adult Abuse Mandatory Reporter Training in state(s) where providing care required within 60 days of hire.  
  • IL Dependent Adult Abuse Mandatory Reporter Training in state(s) where providing care required within 60 days of hire.
