Overview

Medication Therapy Management Pharmacist

UPH Clinics, Waterloo Iowa

Part-time, 16 hours/week

Hours: 8a-6p M-F rotating shifts

 

Provides services including but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Works directly with physicians, other health professionals and patients to provide all aspects of patient care, including clinical and distributive tasks, for patients served within our health system. Helps patients and healthcare professionals achieve clinical outcomes and monitor progress toward common goals. Supervises other pharmacy staff in the absence of formal departmental leadership. Performs assignments and projects as directed by the Pharmacy Leadership Team.

Responsibilities

  • Provide comprehensive medication reviews, medication reconciliation, and patient education to optimize medication therapy outcomes.
  • Collaborate with providers to adjust therapies under Collaborative Practice Agreements and recommend evidence-based treatment plans.
  • Order and monitor appropriate laboratory tests to support safe and effective medication use.
  • Assist patients with medication affordability and adherence strategies.
  • Lead and participate in quality improvement initiatives, workflow enhancements, and standards-of-care development.
  • Educate clinic staff and interdisciplinary teams on medication-related best practices.
  • Maintain compliance with regulatory requirements while demonstrating a commitment to exceptional patient-centered care.

Qualifications

Education

  • Doctor of Pharmacy (PharmD) required.
  • Board Certification in Ambulatory Care, Geriatrics, or a related specialty preferred.

 

Experience

  • 3–5 years of pharmacist practice experience, or completion of a PGY1/PGY2 pharmacy residency.

Licensure & Certifications

  • Current pharmacist license in the state where MTM services are provided.
  • Ability to obtain and maintain pharmacist licensure in additional UnityPoint Health service area states as needed.
