The Direct Support Professional (DSP) Supported Community Living position is responsible for the provision of Home-Based Habilitation skill-based training, support, and/or treatment services to individuals in recovery from a serious mental illness. Services are provided in client homes or community locations. Home-based Habilitation services are designed to assist with the acquisition, retention, or improvement of skills related to living and recreating in the community.

Provides skill-based training, support, and interventions to clients in their home or preferred community location. Assists clients in residing in the most integrated community setting possible based on the member’s needs. Services provided are unique to the individual as identified in their person-centered service plan goals and may include but are not limited to development of daily living skills, symptom management, accessing physical, mental health, and substance use care and treatment, communication, transportation, and social skills.
Recognizes symptoms of physical and mental illness and performs appropriate interventions, communication, and/or referrals in accordance with the identified crisis plan.
Consults with natural and professional supports and community agencies to assist clients with living in the least restrictive setting possible.
Advocates for client rights and choice in obtaining needed services.
Effectively manages client caseload and adjusts schedule as needed to maintain productivity expectations.
Attends regularly scheduled staff meetings, rounding, and in-services.
Participates in and completes all required training by identified due date.
Participates in the planning, implementation, and evaluation of programming effectiveness.
Completes client assessments, social histories, individual service plans, rights restriction reviews, and discharge summaries as needed. Identifies client needs and works collaboratively with client and treatment team to identify goals and appropriate interventions/supports.
Completes documentation of services provided accurately, thoroughly, and timely within established guidelines.
Ensures documentation meets billing requirements and completes necessary corrections timely as required.
Education
High School diploma or equivalency required.
Associate’s degree in human service-related field preferred.
Qualifications/Experience
No experience necessary, 1 year of mental health and/or other disability experience preferred.
