We are seeking (2) Senior Managers- Care Management to join our team. In this role, you'll lead and direct day-to-day care management operations in multiple markets and clinic locations with a complex scope, ensuring performance in panel management, quality, patient experience, and financial outcomes.

• Leads the staffing and workflows of care management RNs, Social Workers and Community Health Workers and oversees scheduling, revenue cycle and budget management, compliance and risk, clinical and operational metrics
• Monitors all operational components and drives continuous improvement to meet organizational objectives
• Partners with clinical and administrative leaders to coordinate services, align resources, implement policies, and support growth across sites
• Collaborates with operations and physician leadership to deliver highly effective and efficient care management services to defined populations
• Serves as a resource across departmental lines for assistance in resolution and/or clarification of issues involving care management

Education: 

• Bachelor's degree in Nursing (BSN) required
• Master's degree in Nursing or Health Care Management or related field preferred

Experience: 

• At least 5 years of leadership experience is requried
• At least 4 years of clinical management or equivalent healthcare management experience is required
• Experience in personnel management, financial management, budgeting, and computer proficiency is preferred
• Care Management or Case Management experience is preferred

Licenses/Certifications:

• RN license required
• Child Abuse Mandatory Reporter Training in state(s) where providing care is requried within 60 days of hire
• Adult Dependent Abuse Mandatory Reporter Training in state(s) where providing care is required within 60 days of hire
