Hospitalist Nurse - Palliative Care

Monday-Friday; 8 hour daytime shifts

We are looking for an experienced RN who is comfortable in Palliative care conversations.  That may include things like disease state management, pain management and controlled substance agreements, and transitions of care discussions.  We are also looking for someone with some critical triage skills and ability to communicate critical information as needed. Through evidence-based practices and a patient-centered approach, you’ll help guide patients and families throughout their healthcare journey. You’ll collaborate closely with providers, nursing staff, social services, patients, and families to support symptom management, advance care planning, appropriate utilization of resources, and positive patient outcomes. Daily opportunities to partner with patients and families navigating complex, chronic conditions.

 

What You’ll Experience

  • Relationship-Driven Care: Build long-term connections with palliative care patients and families, guiding them through diagnosis, treatment, and lifestyle management.
  • Specialized Clinical Growth: Deepen your expertise in palliative care and patient education. Assesses patient and family readiness, confidence, and knowledge related to chronic disease and complex condition self-management.
  • Team-Based Environment: Work alongside physicians, advanced practice providers, pharmacists, social workers, and nurses/care coordinators in both supportive inpatient and  outpatient settings. Collaborates with providers to address medication changes and supports patient understanding of complex medication regimens.
  • Patient Education Leadership: Empower families through hands-on teaching, helping patients and caregivers feel confident managing care at home. Provides education using evidence-based materials, action plans, and Teach-Back methodology.
  • Advance Care Planning (ACP): Supports Advance Care Planning by connecting patients and surrogate decision makers to facilitation resources. Ensures ACP documentation is completed, accurately documented, and readily accessible within the electronic health record.

Responsibilities

 

  • Practice is a mix of inpatient and outpatient: hospital, SNFs, home visits, assisted living facilities, etc.
  • Ability to work as a team member to improve the health of the people and communities we serve
  • Experience with palliative care, medicine management preferred but not required
  • Evaluates, interprets and coordinates information to make decisions and recommendations regarding patient care
  • Patient-focused, friendly and willing to collaborate in a team environment
  • Excellent communication (written and verbal), analytical and problem-solving skills

Qualifications

 

  • Graduate of an accredited program for Registered Nurses.
  • Current license to practice nursing in the State of Iowa/Illinois for Registered Nurse.
  • Meets educational/competency requirements per policy.
  • Ability to use the computer effectively in order to navigate several electronic programs related to patient care.
  • Mandatory Reporter certification.
  • CPR certification.
  • Strong interpersonal skills.
  • Ability to work as a team member.
  • Ability to understand and apply guidelines, policies and procedures.
  • 3-5 years of nursing experience preferred.
  • Bachelor’s education in health related field preferred.
  • Strong verbal and written communication skills.
