The Community Based Services Lead Community Homes position is responsible for overseeing the provision of Home-Based Habilitation services that provide skill-based training, support, and/or treatment to individuals in recovery from a serious mental illness at the Community Home they are assigned. Home-Based Habilitation services are designed to assist with the acquisition, retention, or improvement of skills related to living and recreating in the community. This position reports to the Supervisor of Community Based Services.

Tracks and completes documentation, including progress notes, assessments, social histories, individual service plans, rights restriction reviews, and discharge summaries. Identifies client needs and works collaboratively with client and treatment team to identify goals and appropriate interventions/supports.
Communicates and collaborates with client treatment team members and natural supports via phone, email, or in-person meetings to ensure client needs are met.
Tracks and assists with funding authorizations, client appointments, monthly fire and tornado drills, and client paperwork as required.
Provides and documents direct care when Direct Support Professionals are away from the home, the service requires an additional team member, or when needs are minimal and can be provided by one team member.
Ensures that site cleaning and maintenance schedules are followed, and the location is neat and in good repair at all times.
Is available for after hours, on-call responsibilities as assigned.
Reconciles, orders, and ensures accuracy of the electronic Medication Administrative Record (eMAR).
Provides orientation and training to new staff members in collaboration with the Supervisor Community Based Services and in accordance with identified orientation schedule.
Recognizes and teaches team members the symptoms of physical and mental illness and coaches staff to perform appropriate interventions in accordance with an identified crisis or treatment plan.
Identifies ongoing staff training needs and provides or assigns training as applicable.
Provides clinical consultation, coaching, skill development, and mentoring to staff members working directly with clients.
Schedules, attends, and facilitates regularly scheduled house meetings, client meetings, and staff rounding.
Ensures the timely and accurate completion of progress notes for services provided; performs quality assurance duties for all progress notes completed by staff members.
Assists with audits and program evaluation in accordance with Home Based Habilitation guidelines and Chapter 24 accreditation standards, including but not limited to client charts, transportation record, eMAR, incident reports, etc. Responsible for managing centralized file for incident reports.
Identifies trends and staff training needs related to documentation. Provides necessary training to staff members in collaboration with other Community Based Services Habilitation Leads and the Supervisor Community Based Services.
For individuals with an Associate’s degree in a human service-related field, at least 1 year of experience working in a human-service related field.
For individuals without a degree, at least 2 years of experience working in a human-service related field, must have high school diploma.
Experience working in Medicaid waiver programs.
Prior management/leadership experience preferred. Valid Iowa driver's license required.
