Responsibilities

The Cardiology Electrophysiology (EP) RN Care Coordinator plays a vital role in supporting patients through comprehensive, team-based cardiac care. This position partners closely with physicians, patients, families, and community resources to coordinate care plans, support chronic disease management, improve patient outcomes, and provide education related to cardiac conditions, medications, procedures, and lifestyle modifications. The ideal candidate is a compassionate, organized nurse with strong clinical judgment and a passion for delivering high-quality cardiovascular care. Cardiology or Electrophysiology (EP) experience is strongly preferred.

  • Coordinate and manage patient-centered care for cardiology and EP patients across the continuum of care
  • Collaborate with physicians, care teams, home health agencies, and community resources to support optimal patient outcomes
  • Provide patient and family education regarding cardiac conditions, medications, procedures, diet, exercise, and self-management strategies
  • Support patients during transitions of care through follow-up communication and care coordination
  • Identify high-risk patients and assist with interventions to improve quality outcomes and reduce unnecessary utilization
  • Utilize critical thinking and clinical assessment skills to support individualized care planning
  • Serve as a patient advocate and resource for patients, families, and interdisciplinary team members
  • Participate in quality improvement initiatives and population health management activities
  • Maintain accurate documentation and compliance with organizational, HIPAA, OSHA, and regulatory standards

Qualifications

Required

  • Graduate of an accredited Registered Nursing program
  • Current RN license in the state of practice
  • Current BLS certification
  • Strong communication, organizational, and critical thinking skills
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Proficiency with computer systems and electronic medical records

Preferred

  • Previous cardiology or Electrophysiology (EP) experience
  • Prior care coordination or case management experience
  • Experience in a medical office or outpatient clinic setting
  • Bachelor of Science in Nursing (BSN)
  • Process improvement or population health experience
