As a member of the interdisciplinary team, assists with individual or group education and exercise. Works alongside other team members to develop appropriate treatment plans.

Responsibilities

Clinical Excellence/Patient Care

  • Assesses the needs of patients and families, providing appropriate teaching and making the necessary referrals to provide continuity of care.
  • Utilizes scientific principles to develop exercise prescriptions. Collaborates with patient/family and the healthcare team to develop an individual plan of care. Monitors client’s response to exercise and adjusts treatment plan accordingly
  • Promptly completes documentation to assure an accurate legal record of patient’s care.
  • Delivers high quality care while demonstrating knowledge and clinical skills of the specialty required on assigned unit in accordance with established policies, procedures and standards of care.
  • Utilizes holistic approach (considering biopsychosocial, spiritual, educational, safety, and related criteria) appropriate to the individuals served.
  • Demonstrates initiative to improve both individual and department level quality and customer satisfaction.
  • Delegates and provides direction for other healthcare providers and holds them accountable for their involvement in the plan of care
  • Effective at directing/supervising group & individual in-patient or out-patient education/exercise
  • Utilizes critical thinking in making independent judgments, and maintains responsibility and accountability for the knowledge of conditions of assigned patients.
  • Specialized skills to include, but not limited to, obtaining and interpreting EKG-12 lead, knowledge of common cardiac, pulmonary, and emergency medications, interpretation of rhythms/arrhythmias, telemetry application/use, lab value interpretation, cardiac & pulmonary specific equipment (pacemaker/defibrillators, LVADs, oxygen/portable ventilators, etc.), exercise prescription and progression principles, incision care (CABG, angiogram, transplant, etc.), accurately charge and track unit statistics, & knowledge/practice unit specific policies.

 

Clinical Excellence and Patient Education

  • Assess and chooses teaching strategies appropriate to the patient’s and their families’ specific needs, culture, gender, readiness to learn, and level of comprehension.
  • Consults appropriate resources within the hospital to provide specialized education.
  • Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.

Qualifications

  • Education: Undergraduate degree in Exercise Science or related field required.
  • Experience: Previous experience in a cardiac rehabilitation setting preferred, including risk factor counseling and individualized goal setting preferred.
  • License/Certifications: Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.
  • Knowledge/Skills/Abilities: Writes, reads, comprehends and speaks fluent English. Multicultural sensitivity. Microsoft Office – basic computer skills. Customer/patient focused. Critical thinking skills using independent judgment in making decisions. 
  • Other:  Use of usual and customary equipment used to perform essential functions of the position.
