Shift: Full-time, Days, Monday - Friday, for 80 hours per pay period. 

 

Make a direct impact on patients’ independence and quality of life as a Home Care Physical Therapist. In this role, you will provide evidence-based therapy in patients’ homes, conduct comprehensive evaluations, and develop individualized care plans in collaboration with physicians and interdisciplinary teams. You will deliver skilled interventions, educate patients and caregivers, and ensure accurate documentation, all while driving meaningful functional improvement, safety, and positive patient outcomes in a compassionate home setting.

Responsibilities

  • Provide skilled physical therapy care through in-home visits, conducting comprehensive assessments and developing individualized treatment plans to restore function and improve patient independence. 
  • Deliver evidence-based interventions including therapeutic exercise, pain management, and rehabilitative techniques aligned with the plan of care. 
  • Evaluate patient progress at each visit, update care and discharge plans, and ensure goals are measurable, realistic, and patient-centered. 
  • Educate patients, caregivers, and families on disease management, safe mobility, equipment use, and strategies to prevent complications and hospitalizations. 
  • Coordinate closely with physicians and interdisciplinary care teams, communicating changes in condition and ensuring timely updates to the care plan. 
  • Maintain accurate, timely clinical documentation in compliance with CMS, payer requirements, and agency standards, including OASIS data collection. 
  • Supervise home health aides and therapy assistants while ensuring safe, effective, and high-quality care delivery. 
  • Participate in quality improvement, peer review, case conferences, and care coordination activities to enhance outcomes and reduce avoidable utilization. 
  • Manage patient assignments independently, prioritize workload effectively, and maintain productivity standards in a dynamic home health environment. 
  • Foster strong professional relationships while upholding regulatory compliance, safety standards, and best practices in home-based care. 

Qualifications

Education: 

  • Graduate of a Physical Therapist education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to U.S. physical therapist entry-level education by a credentials evaluation organization approved by the American Physical Therapy Association required.  
  • Master’s Degree in Physical Therapy preferred. 
  • Doctor of Physical Therapy (DPT) preferred. 

Experience: 

  • One (1) year prior experience caring for similar patient population as to be assigned required.  
  • Two (2) years of home care experience preferred.  
  • One (1) year prior experience with documentation in an electronic health record preferred. 

License(s)/Certification(s): 

  • Licensed Physical Therapist in state(s) of practice required. 
  • Valid driver’s license required. 
  • Valid Auto Insurance required. 
  • Basic Life Support through the American Heart Association required. 
  • Certificate for OASIS Specialist-Clinical preferred. 
  • Certified in one (1) or more areas applicable to the home care population or specialty practice preferred: 
    • Assistive Technology Professional (ATP)  
    • Seating and Mobility Specialist (SMS)  
    • Neuro-Developmental Treatment Certification (C/NDT)  
    • Certified Aging in Place Specialist (CAPS)  
    • Certified Diabetes Care and Education Specialist (CDCES)-CBDCE  
    • Certified Hand Therapist (CHT)  
    • Certified Lymphedema Therapist (CLT)  
    • Certified Low Vision Therapist (CLVT)  
    • Certified Neuro Specialist (CNS)  
    • Lee Silverman Voice Treatment- BIG (LSVT BIG)  
    • Board Certified Geriatric Clinical Specialist  
    • Board Certified Cardiovascular and Pulmonary Clinical Specialist  
    • Board Certified Neurologic Clinical Specialist  
    • Board Certified Orthopedic Clinical Specialist  
    • Board Certified Oncologic Clinical Specialist

    • Advanced Competency in Home Health (ACHH)
