Overview

RN Navigator - Cardiovascular Center

Waterloo, IA

Full-time, 40 hours/week

8 hour shifts

 

The Navigator develops and coordinates the overall plan of care for a cohort of patients throughout their hospital stay and beyond (as directed), establishing strong partnerships with patients, families, physicians and all care givers. Assumes a broad level of accountability for the outcomes of care for assigned patients and provides leadership to the multi-disciplinary team.

Responsibilities

 

The Navigator develops and coordinates the overall plan of care for a cohort of patients throughout their hospital stay and beyond (as directed), establishing strong partnerships with patients, families, physicians and all care givers. Assumes a broad level of accountability for the outcomes of care for assigned patients and provides leadership to the multi-disciplinary team.

Qualifications

  • Current license in the state of Iowa or meets criteria established by Iowa Board of Nursing.
  • Certification in specialty preferred; commitment to obtain within 2 years of hire date.
  • BSN required or actively enrolled in BSN completion program. MSN preferred.
  • 3 years recent experience in direct patient care involving specialty.
  • Team leadership skills and excellent communication skills.
  • Able to meet the physical demands on file in the Human Resources Department.
