The Home Medical Equipment Coordinator plays a key role in ensuring timely, accurate coordination of patient referrals and reorders for medical equipment, supplies, and services. In this fast-paced position, you will serve as a central communication link between referral sources, patients, and the internal operations team, helping to streamline processes and support high-quality patient care. Responsibilities include verifying insurance coverage, obtaining authorizations, and ensuring all orders meet payer and regulatory requirements. Success in this role requires strong attention to detail, organizational skills, and a proactive approach to facilitating efficient equipment setup and an exceptional patient experience.

· Maintains a broad knowledge in all UnityPoint at Home service lines including, Home Health, Hospice and Infusion services.
· Routinely monitors for new referrals via fax, EMR and other electronic applications.
· Accepts all new referrals and initiates referral documentation in the EMR and ensures accurate information and documentation is included in patient record to facilitate the admissions process, which includes diagnosis codes after validating with RN.
· Secures, confirms and accurately documents provider orders in a timely manner.
· Operates computer system to record/document and retrieve information to ensure appropriate agency services are provided in a timely manner.
