We are seeking a Social Worker to join our interdisciplinary team, applying social work expertise to support patient and family access to care and address psychosocial factors impacting health outcomes.

Responsibilities

Patient Care & Support

  • Conducts psychosocial assessments to identify needs, strengths, and barriers to care.
  • Provides crisis intervention, counseling, and advance care planning support.
  • Connects patients and families with community resources, services, and financial supports.
  • Assists with care transitions and completes required screenings (e.g., PASRR).

Advocacy & Education

  • Advocates for patient rights, safety, and autonomy, including reporting concerns as required.
  • Educates patients, families, and care teams on psychosocial aspects of care and available resources.
  • Supports culturally competent care, including coordination of interpreter services.

Care Coordination

  • Collaborates with the interdisciplinary team to develop and support patient-centered care plans.
  • Facilitates team and family meetings to promote communication and continuity of care.
  • Partners with internal and community providers to ensure seamless care across settings.

Qualifications

Education:

Bachelor’s degree in social work from an accredited school of social work


Experience:

Prefer one year clinical experience in healthcare

 

License(s)/Certification(s):

Current licensure in good standing to practice in the state where services are provided (BSW)

Valid Mandatory Reporter course completion by state requirement

Completion of First Steps (ACP) and IPOST/POLST training

LMSW or LISW preferred.
