The Rehabilitation Referral Coordinator plays a vital role in delivering high-quality, cost-effective care to patients undergoing acute rehabilitation. This individual serves as a liaison among patients, rehabilitation teams, and external resources, ensuring seamless coordination of services. They are responsible for conducting pre-admission intake assessments for acute inpatient rehabilitation, ensuring compliance with admission criteria, CMS regulations, and IRF standards. Acting as a primary facilitator for patient referrals to the Inpatient Rehabilitation Facility (IRF), they engage with discharge planners, insurance carriers, physicians, patients, and family members, fostering strong communication and collaboration. The coordinator works closely with leadership to maintain standards, including reporting and interpreting rehabilitation data, preparing required reports, and ensuring compliance with CMS and IRF requirements. As an integral part of the multidisciplinary rehabilitation team, they contribute to a collaborative effort aimed at optimizing patient outcomes.
