• Collaborate closely with neurologists, advanced practice providers, and interdisciplinary teams to develop and implement individualized care plans.
• Facilitate transitions of care, including hospital follow-ups and specialty referrals.

Patient & Caregiver Education
• Provide in-depth education on neurological diagnoses, treatment plans, medications, and symptom management strategies.
• Support patients and families in navigating complex conditions such as epilepsy, multiple sclerosis, migraines, and neurodegenerative diseases.
• Promote patient self-management, safety, and adherence to care plans.

Clinical Documentation & Outcomes Management
• Maintain accurate and timely documentation of all patient interactions, assessments, and care coordination activities in the electronic health record (EHR).
• Monitor patient progress, track outcomes, and ensure appropriate follow-up care to support continuity and quality outcomes.
• Participate in quality improvement initiatives related to neurological care delivery.
