Under the direction and supervision of the PACE Business Manager, coordinates the intake and enrollment processes for PACE. Assists in promotion/marketing of PACE to community organizations.
Program Management
• Oversees coordination of the intake and assessment processes.
• Assesses potential participants in home, nursing home, hospital or other settings as part of the intake process.
• Maintains current, written assessment and intake records inclusive of potential participants expressed wishes and the referral services provided, as appropriate.
• Collects data in accordance with PACE requirements; prepares reports as required by the programs.
• Assists in evaluating the intake and assessment process, identifying problem areas accordingly and making changes as recommended.
• Assists with Medicaid certification process in cooperation with Iowa Medicaid Enterprise and Iowa Department of Human Services.
• Adheres to PACE Participants Rights Policy at all times.
• Protects privacy and maintains confidentiality of participant employee, and sensitive agency information.
Relationship Building
• Contacts identified key referral sources regularly to discuss possible program candidates.
• Maintains ongoing relationships with identified individuals and agencies to ensure a clear understanding of PACE as a vital community resource.
• Provides information and referral as appropriate to inquiries.
• Provides alternative community resources to applicants not accepted into PACE
• Shares responsibility with other team members in the implementation of PACE philosophy and achievement of program goals.
Education
Bachelor’s Degree in Social Services or health related field OR a combination of education and experience
Experience
Demonstrated experience of strong communication skills, including face-to-face, telephone, and computer skills
Demonstrated strength in attention to detail and organization
License(s)/Certification(s)
Valid Driver’s License
Valid Auto Insurance
Maintain Mandatory reporter status
Knowledge/Skills/Abilities
Ability to understand and navigate State of Iowa health and Social Service delivery systems, and aging network. General knowledge of financing mechanisms, such as Medicare, Medicaid, Prospective Payment Systems, HMO Managed Care providing risk-based capitated systems. Able to assess potential participant’s financial status.
