Responsibilities

The PACE Registered Nurse (RN) provides skilled nursing care to PACE participants through home and community-based visits. The RN conducts assessments, delivers nursing interventions, coordinates care, and collaborates with the interdisciplinary team to support participants in remaining safely in their homes while maintaining optimal health and independence.

 

Key Responsibilities:

  • Conduct comprehensive nursing assessments during home and community visits

  • Develop, implement, and evaluate individualized nursing care plans in collaboration with the interdisciplinary team

  • Perform skilled nursing services including medication management, wound care, injections, vital signs monitoring, and chronic disease management

  • Assess changes in participant condition and initiate appropriate interventions or referrals

  • Educate participants and caregivers on disease processes, medications, treatments, and safety

  • Coordinate care with physicians, therapists, personal care attendants, and social services

  • Participate in care plan meetings and interdisciplinary team conferences

  • Ensure compliance with PACE, Medicare, Medicaid, and state regulatory requirements

  • Document assessments, interventions, and outcomes accurately and timely in the medical record

  • Promote infection control, fall prevention, and participant safety

  • Support transitions of care, including hospital discharges and follow-up visits

  • Serve as a clinical resource for PCAs and other care team members

Qualifications

Education

Graduate of State Board approved program for Registered Nurses
Experience

One year nursing experience caring for similar patient population as to be assigned.
License(s)/Certification(s)

Valid license as Registered Nurse in state where providing care.

Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
Mandatory Reporter: Child & Dependent Adult Abuse.

Completion of PACE competency validation activities.

CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
