The Registered Dietitian (RD/RDN) provides comprehensive nutrition assessment, care planning, and ongoing nutrition management for PACE participants. As a member of the interdisciplinary team, the Dietitian supports participant health, functional independence, and quality of life through individualized, culturally appropriate nutrition interventions.
Conduct comprehensive nutrition assessments for PACE participants, including medical history, dietary intake, lab values, and functional status
Develop, implement, and monitor individualized nutrition care plans in collaboration with the interdisciplinary team
Provide nutrition counseling and education to participants and caregivers
Manage therapeutic diets related to chronic conditions such as diabetes, renal disease, cardiovascular disease, and dysphagia
Recommend and monitor oral nutrition supplements and enteral nutrition as indicated
Participate in interdisciplinary care plan meetings and case conferences
Monitor participant weight, nutritional status, and risk for malnutrition
Coordinate with food service providers and adult day health centers to ensure diet compliance and quality
Support safe swallowing practices in collaboration with speech therapy as needed
Ensure compliance with CMS PACE regulations, Medicare, Medicaid, and state requirements
Document nutrition assessments, interventions, and outcomes accurately in the medical record
Provide staff education related to nutrition, special diets, and food safety
Participate in quality improvement initiatives and regulatory audits
Education
Bachelor of Science degree in Dietetics and/or food and nutrition that meets or exceeds the requirement by the American Dietetic Association for the registration examination.
Successful completion of internship or coordinated undergraduate program that meets the American Dietetic Association guidelines.
License(s)/Certification(s)
• Registration or registration eligible with the Commission on Dietetic Registration
• Licensed or licensed eligible with the State of Iowa
• Current CPR/BCLS certification
• Maintains current Adult Mandatory Reporter status
• Maintains American National Standards Institute accredited food protection/safety certification (i.e. ServSafe Food Protection Manager/Certified Food Safety Manager) or obtained within 90 days for new hire.
• Valid driver’s license
• Valid Auto insurance
