We are seeking a compassionate clinical professional to join our team. This position is open to Certified Medical Assistants (CMA), OR Licensed Practical Nurses (LPN). This clinical professional will provide support in patient care, assisting with exams, treatments, and patient education.
Assist providers and nurses with participant care in clinic, adult day health center, and community settings
Obtain and document vital signs, height, weight, and health histories
Prepare participants for medical examinations and procedures
Perform clinical tasks within scope of practice, such as EKGs, phlebotomy, specimen collection, and injections (as permitted by state law)
Support medication management activities, including medication reconciliation and refills under supervision
Assist with wound care and dressing changes as directed
Maintain exam rooms, equipment, and supplies in a clean and safe manner
Accurately document clinical information in the electronic health record (EHR)
Coordinate appointments, referrals, and follow-up care as assigned
Communicate participant concerns or changes in condition to the nursing or medical team
Follow PACE policies, infection control standards, HIPAA regulations, and safety protocols
Participate in interdisciplinary team meetings, training, and in-service education
Education
• Graduate of an accredited nursing program or current CMA certification.
• High school diploma or equivalent.
License(s)/Certification(s)
• Current CMA certification OR Current LPN license
• BLS and Mandatory Reporter certification.
• Valid driver’s license when driving any vehicle for work-related reasons.
