Plans, coordinates, and evaluates healthcare needs with patient, multidisciplinary healthcare professionals, and patient’s support system as directed by the medical staff and following the policies and procedures of UPH-Grinnell Regional Medical Center. Participates with developing care pathways for effective management and use of resources. Coordinates standards of care and expected outcomes within appropriate utilization guidelines. Objectively assesses and individualizes care throughout hospitalization and discharge to ensure post-hospital care and referrals are appropriate for patients’ disease process.

Care Coordination
• Coordinates with multidisciplinary team to begin assessments, establish goals, and plan the client’s continuum of care through discharge.
• Assists direct caregivers in completion of the nursing processes including teaching, daily tasks, and special projects.
• Maintains responsibility for continuum of care of assigned patients, delegating only those tasks that are within the healthcare providers designated scope of practice.
• Formulates and updates individualized care plan and teaching plans in cooperation with direct caregivers and evaluate outcomes.
• Assesses care concerns and goals with patients and their families to identify understanding and concerns as it relates to client’s disease process.
• Performs on-call duties on weekends as assigned.
• Assists with determining admission and ongoing status of patients.
• Evaluates swing bed referrals, establishes patient goals, and completes and submits MDS forms in a timely manner.
Quality of Care
• Performs Quality and Utilization Review data collection and activities as assigned.
• Monitors severity of illness and intensity of service to ensure appropriateness of admissions and contacts insurance providers.
• Monitors readmissions and implements processes to prevent readmissions through effective transition to other level of services or home setting.
• Assists with policy and forms development to comply with regulatory and documentation requirements.
Education:
• Graduate of an accredited Nursing Program required
• Bachelor’s degree in Nursing preferred
Experience:
• Minimum of three years’ experience in acute care nursing required
• Current knowledge of discharge planning and utilization review activities preferred
License(s)/Certification(s):
• Current Iowa license to practice as a Registered Nurse required
• Valid driver’s license when driving any vehicle for work-related reasons required
