The Clinic RN provides direct nursing care and clinical support within the PACE primary care clinic. This role ensures safe, timely, and person-centered care for frail older adults enrolled in the Program of All-Inclusive Care for the Elderly. The Clinic RN supports providers, performs clinical procedures, coordinates participant care, and communicates effectively with the interdisciplinary team (IDT).

Provide ambulatory nursing care for PACE participants during clinic visits.
Perform skilled nursing tasks such as wound care, injections, catheter care, EKGs, vitals, and medication administration.
Assist with urgent triage and respond to participant concerns during clinic hours.
Support primary care providers with assessments, procedures, and participant follow-up.
Coordinate referrals to specialists, diagnostic testing, and therapies.
Communicate abnormal results or significant changes in participant condition to providers and the IDT.
Ensure safe clinic flow by rooming participants, preparing equipment, and maintaining infection control standards.
Assist with medication reconciliation, chronic disease monitoring, and care plan updates.
Maintain clinic inventory, ensure proper storage of medications, and monitor supplies and equipment.
Document all nursing interventions, assessments, and communications in the electronic health record accurately and in a timely manner.
Help track and monitor immunizations, preventive screenings, and chronic care management metrics.
Work closely with physicians, nurse practitioners, social workers, rehabilitation therapists, dietitians, and home care staff.
Participate in daily team huddles and collaborate on participant care plans
Education
• Graduate of an accredited program for Registered Nurses.
• Current license to practice nursing in the State of Iowa for Registered Nurse.
• High school diploma required if work is performed in a complex CLIA lab.
