Responsibilities

• Interacts with patients, families, visitors and team members in a positive and supportive manner with sensitivity to cultural differences.
• Help patients overcome barriers to services which include, but are not limited to anxiety, financial deficits, transportation issues, health limitations, etc.
• Helps patients get connected to community resources
• Assist patients in completing patient assistance and other public assistance forms
• Assist patients in completing health questioners
• Provide direct services to patients as needed including but to in home services
• Respect patient confidentiality and interact with others in a professional manner
• Makes monthly contact with patients enrolled in IHH through phone or face to face contact to fill any gaps in care that are identified.
• Consults with members of the healthcare team and provides updates and other information to patients and families.
• Gather, verify and enter demographic and insurance information on new and established patients necessary for completion of electronic health record and insurance claim processing at each patient visit on patient accounting system.
• Demonstrates effective and caring communication skills and telephone techniques and etiquette.
• Document on contact with patients in EPIC. 

Qualifications

Education:

High School Diploma or GED required
Associates degree in Social Work, Human Services, or another Human Service related field preferred

 

Experience:

Must have lived experience with one own mental health and in a state of recovery required

Experience with providing service to individuals with a severe and persistent behavioral health needs preferred

 

License(s)/Certification(s):

Valid Class D driver’s license when driving any vehicle for work-related reasons. Must be 21 years of age.

Completed or will complete a state sanctioned Peer Support Specialist training no later than 6 months after one’s start date
Certified Mental Health Peer Support Specialist or Certified Peer Recovery Specialist through Iowa Board of Certification or will complete within 12 months of hire

 

Knowledge/Skills/Abilities:

Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively. Individual will demonstrate knowledge and skills necessary to provide appropriate care to adult and geriatric patients with severe and persistent mental illnesses. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills.

 

Other:

Use of usual and customary equipment used to perform essential functions of the position. Position requires working remotely, in community and/or from home. Must have access to cell phone use during working hours.
