Responsible for all aspects of Pharmaceutical care provided at the Hospital: Evaluates medication orders at verification to promote rational cost effective drug therapy, Supervises accurate dispensing of product to positively impact patient outcome, Supervises technical staff to assure safe and accurate completion of Unit Dose and IV admixture services, Supervises medication storage areas both within and outside pharmacy to assure drug safety and security, Provides patient education about new drugs and potential adverse reactions to drugs as requested, Assists with Medication reconciliation activities, Applies all Pharmacy related laws and required documentation, and Documents interventions in appropriate system. Also provides Renal dosing program, kinetic & dosing service, anticoagulation monitoring and dosing service as well as assistance precepting Pharmacy Residents as requested/scheduled. Provides these services to neo-natal, pediatric, adolescent, adult, and geriatric customers.

Patient Care Distributive Duties
Basic UPH Performance Criteria
Education: Graduate of accredited College of Pharmacy
Experience: Prefer one (1) year of hospital clinical experience, Residency
License(s)/Certification(s): Current licensure in good standing to practice as a Registered Pharmacist in Iowa or licensure eligible in Iowa. Valid driver’s license when driving any vehicle for work-related reasons. Completes at least 2 hours of stroke education CE annually.
Knowledge/Skills/Abilities: Writes, reads, comprehends & speaks English. Multicultural sensitivity. Preparation of IV Sterile Products Customer/patient focused.
Critical thinking skills using independent judgment in making decisions.
Other: Use of usual and customary equipment used to perform essential functions of the position. Work may occasionally require travel to other UPH facilities. May drive a UPH vehicle, rental or own vehicle.
