The RN Care Coordinator plays a vital role in ensuring seamless, patient-centered care for individuals with acute and chronic pulmonary conditions. This position combines clinical expertise with care management and patient advocacy to support patients through the continuum of pulmonary care—from diagnosis and treatment to follow-up and disease management. The RN Care Coordinator serves as a key liaison between patients, families, providers, and community resources to promote optimal health outcomes and improve the overall patient experience.
Coordinate comprehensive care for patients with chronic respiratory illnesses such as COPD, asthma, interstitial lung disease, and sleep disorders.
Perform clinical assessments and develop individualized care plans in collaboration with pulmonologists and advanced practice providers.
Facilitate communication among the care team, patients, and families to ensure continuity of care across inpatient, outpatient, and community settings.
Manage and track follow-up care, diagnostic testing, medication refills, and treatment adherence.
Provide education and counseling on disease management, medication use, smoking cessation, oxygen therapy, and lifestyle modifications.
Support transitions of care following hospitalizations or emergency visits, ensuring timely follow-up and preventing readmissions.
Assist with prior authorizations, insurance coordination, and documentation of clinical encounters in the electronic health record (EHR).
Monitor quality metrics and participate in quality improvement initiatives related to pulmonary health outcomes.
Serve as a resource and advocate for patients navigating complex care systems.
Current, active Registered Nurse (RN) license in the State of Iowa
Graduate of an accredited nursing program (ADN or BSN)
Basic Life Support (BLS) certification
Strong organizational, communication, and critical-thinking skills
Bachelor of Science in Nursing (BSN) - preferred
Minimum 2 years of clinical experience in pulmonology, internal medicine, or chronic disease management - preferred
Case management or care coordination experience - preferred
Familiarity with Epic or other electronic health record systems - preferred
Certified Case Manager (CCM) or Care Coordination certification (or willingness to obtain) - preferred
