Shift: Monday - Friday | 8:00 AM - 5:00 PM (+ weekend/call)

The RN Admission is the first point of clinical contact for patients entering Homecare services—setting the tone for a high-quality, compassionate care experience. Using expert clinical judgment, hands-on assessment skills, and a strong understanding of home health needs, this nurse initiates individualized plans of care in collaboration with the physician. By coordinating with the interdisciplinary team, the RN Admission ensures patients receive timely, effective care that leads to better outcomes and a smooth transition into home-based healing.

• Performs initial and comprehensive assessments on patients in accordance with agency policies and procedures.
• Promotes the services of UnityPoint at Home internally and externally.
• Ensures organizational sustainability through the efficient use of corporate resources.
• Case manages as needed or as requested by Supervisor.
• Ensures quality and safe delivery of services to patients according to the physician’s orders.

Education:

• Graduate of State Board approved program for Registered Nurses

Licensure/Certifications:

• Valid license as RN in state(s) where providing care.
• Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
• Mandatory Reporter: Child & Dependent Adult Abuse.
• CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

Minimum Experience:

• One year nursing experience, including experience in performance of OASIS, in skilled home health care

Other:

• Use of usual and customary equipment used to perform essential functions of the position.
