M-F 8am-5pm

 

Responsible for all aspects of pharmaceutical care provided at the Cancer Center. Evaluates medication orders at verification to promote rational cost-effective drug therapy. Supervises accurate dispensing of medications to positively impact patient outcome. Supervises technical staff to assure safe and accurate completion of Unit Dose and IV admixture services. Supervises medication storage areas both within and outside pharmacy to assure drug safety and security. Provides patient education about new drugs and potential adverse reactions to drugs as requested. Assists with Medication reconciliation activities. Applies all Pharmacy related laws and required documentation. Documents interventions in appropriate system.

Responsibilities

Patient Care Duties:

  • Verifies all orders generated on shift, keeps work flow up to date and current, and cleans up old orders as soon as possible.
  • Reviews patient treatment plans, verify the drug calculation, labs, dosing timing and potential drug interaction prior to mixing.
  • Completes verbal hand off communication to next shift, to ensure continuity of quality patient care and follow through in interventions.
  • Screens and affirms appropriateness of orders including allergy check, formulary compliance, pregnancy & lactation status, dosing, duplicate orders etc.
  • Analyzes drug treatment regimens for potential adverse drug reactions, drug interactions, or contraindications. Assesses the clinical significance of such events, identifies any drug-related problems that may be affecting the patient.
  • Provides quality patient specific pharmaceutical care to patients and documents such interventions with respect to clinical outcomes and financial ramifications.
  • Fields and investigates drug information questions.  Uses resources appropriately and colleagues as needed to ensure timely and accurate responses.  Documents as necessary.
  • Maintains good rapport with peers, nursing staff, physicians and other departments.
  • Participates in order set development and drug treatment pathways.
  • Assists with training: Students, Technicians, and new pharmacist’s orientation.  Coaches and instructs in proper procedures, focusing on quality outcomes.

Qualifications

  • Education:
  • Graduate of accredited College of Pharmacy required
  • Pharm D preferred
  • Pharmacy Residency preferred
  • Experience:
  • 1 year experience in hospital clinical pharmacy preferred
  • Pharmacy residency preferred
  • License(s)/Certification(s):
  • Current licensure in good standing to practice as a Registered Pharmacist in Iowa or licensure eligible in Iowa required
  • Board of Pharmacy Specialties Certification preferred
  • ACLS/PALS preferred
