Overview

The Licensed Practical Nurse (LPN) is a nurse who works in the wound center in collaboration with the wound center provider and other wound care team members to achieve optimal patient outcomes. They have acquired knowledge, judgement, and skills specific to working in that role as a Licensed Practical Nurse in the wound center.

Responsibilities

Assessment:
• Perform follow-up nursing assessment of patient's conditions and report findings to RN.
• Make reports available to the assigned case manager and physician.
• Recognize alterations in the patient's healing progress and verbalize this to other members of the healthcare team.
• Organize assessment data for accuracy, completeness and confidentiality.
• Record wound assessment data in an orderly fashion and communicate, revise and verbalize pertinent information to other health care team members.
• Assess, stage and measure wounds accurately and in accordance to policy.
• Photograph wounds according to policy and procedure.
• Identify patient care situations that require intervention, implement nursing actions identified by Registered Nurse (RN)/case manager, and evaluate with RN/case manager responses.
• Identify patient/family education and discharge needs and implement with RN/case manager a teaching plan of care.

Planning:
• Collaborate with RN to identify the patient’s present and potential problems from the assessment.
• Implement a teaching plan in collaboration with RN/case manager.
• Prioritize problems according to impact on health status
• Formulate with case manager the desired outcomes that are specific and measurable within a certain time frame and consistent with other health provider's expectations.
• Identify desired outcomes of home healthcare needs, support system evaluation and patient teaching needs from admission to discharge. Communicate information to case manager.

Implementation:
• Participate in implementing planned changes and activities to improve care.
• Hold self and staff accountable for the delivery of quality nursing care.
• Promote harmonious relationships and favorable attitudes among the team.
• Implement wound care and hyperbaric oxygen (HBO) protocols when clinically indicated.
• Individualize care according to age specific population.
• Assist physician with wound therapy.
• Accurately complete the super bill and ensure that all orders and charges are initiated accurately.
• Coordinate activities with other disciplines (rehab, nutrition, diabetic education, home health, nursing homes and other ancillary services).

Function as an intake/discharge nurse during clinic times:
• Coordinate office visits, pre-procedure work-ups and patient education with the multidisciplinary team.
• Foster continuity of care between Wound Healing Center (WHC) and caretaker (home, hospital, Home Health etc.).
• Participate in coordination of the use of clinic staff, exam room space, and supplies.
• Prepare for clinic to enable accurate and complete updates to the Medical Director.
Maintain skills and knowledge through training and continuing education:
• Complete mandatory education requirements.
• Attend staff meetings and in-services.
Perform completely in defined clinic roles a variety of routine procedures:
• Monitor condition of equipment and report malfunctions to Clinical Manager.
• Safely operate all equipment in the clinic.
• Maintain appropriate infections control procedures.

Qualifications

Education:

LPN or Associate Degree Nursing required.
Bachelors of Science in Nursing preferred.

 

Experience:

Broad-based clinical experience preferred.

 


License(s)/Certification(s):

Current RN Licensure in State of Iowa or applicable compact state. Must be licensed in state of residence

Case Managers must attend 1 week training course provided by Healogics prior to working in role. Basic life Support certification.

Valid driver’s license when driving any vehicle for work-related reasons.
